Immunization Resources

New York State Public Health Law § 2165 requires all students born on or after January 1, 1957 who are enrolled for at least six (6) credits or more to provide proof of immunization to measles, mumps, and rubella. New York State Public Health Law § 2167, which requires the distribution of information regarding meningococcal meningitis and meningitis immunization to all students who are enrolled for at least six (6) credits or more and completion by such students of a meningitis response form and proof of immunization to meningococcal meningitis where applicable.

All international immunization documents must be translated into English prior to submission by a professional translation service.

COVID-19 vaccination is no longer required, but students are encouraged to stay up to date with their COVID-19 vaccination.

Measles, Mumps, and Rubella

All students born on or after January 1, 1957 enrolled for at least six (6) credits or more shall be required to provide the Academy with certification from a health care provider or other acceptable evidence that they have received immunization against measles, mumps and rubella with vaccines that meet the standards for such biological products that have been approved by the United States Public Health Service and the New York State Department of Health; or evidence of immunity by history of disease or serological evidence.

A student may meet such requirement by providing the documentation set forth under either Section (1) or (2) below:

  1. A certificate of immunization that shows the student has:

a.  With respect to measles, either:

i.  received two doses of live measles virus vaccine, the first dose administered after the age of 12 months and the second dose administered more than 30 days after the first dose but after 15 months of age; or

ii.  been diagnosed by a physician as having had measles disease; or

iii.  demonstrated serological evidence of measles antibodies by submitting a lab report; or

iv.  if the student is unable to provide a certificate of immunization that satisfies the requirements in (i), (ii) or (iii) above, documentation that proves the student attended primary or secondary school in the United States after 1980, and a certificate of immunization that documents a dose of measles vaccine was administered within one year prior to attendance at the post-secondary institution; and

b.    With respect to rubella, either:

i.  received a single dose of live rubella virus vaccine administered after the age of 12 months; or

ii.  demonstrated serological evidence of rubella antibodies by submitting a lab report; and

c.    With respect to mumps, either:

i.  received a single dose of live mumps virus vaccine administered after the age of 12 months; or

ii.  been diagnosed by a physician as having had mumps disease; or

iii.  demonstrated serological evidence of mumps antibodies by submitting a lab report.

 

2.   A certificate of immunization that shows the student is in the process of completing the requirements of Section (1) above and:

a.  has received at least one dose of live measles virus vaccine as required in Section (1); and

b.  has complied with the requirements in Section (1) for mumps and rubella; and

c.  has an appointment to return to a health practitioner for the remainder of the immunizations specified in Section (1), if this appointment is scheduled no more than 90 days since administration of the first dose of measles virus vaccine.

 

Proof of discharge from the armed services within ten (10) years from the date of application to the Academy shall enable a student to attend courses pending actual receipt of armed forces records indicating such student has been immunized against measles, mumps, and rubella.

 

Meningococcal Meningitis

All students enrolled for at least six (6) credits or more shall receive written information about meningococcal meningitis and meningitis immunization, and complete a meningococcal meningitis response form,  certifying either that they have received such immunization within the past five years, or that they have received the written information from the Aacdemy and understand the risks of meningococcal meningitis but have chosen not to be immunized against it.

All students enrolled for at least six (6) credits or more must provide the University with certification from a health care provider or other acceptable evidence that they have received immunization to meningococcal meningitis A,C,W,Y within the prior 5 years.

 

In Process

A student is considered in process and allowed to attend classes if he/she has presented a Certificate of Immunization that shows the student is in the process of completing the immunization requirements of PHL Section 2165.

To be “in process” the student must have received at least one dose of live measles virus vaccine, have complied with the requirements for mumps and rubella, and have an appointment to return to a health practitioner for the second dose of measles if this appointment is scheduled no more than 90 days since administration of the first dose of measles virus vaccine.

A student can be considered in process of complying with PHL Section 2167 regarding meningococcal disease until the maximum 30 day grace period has elapsed. The 30 day grace period may be extended to 60 days if a student can show a good faith effort to comply with PHL Section 2167. If a student is granted the extended grace period, then exclusion begins immediately after the 60 days elapses.

 

Exemptions from Immunization Requirement

MEDICAL EXEMPTION

Medical Exemption: If a licensed physician, physician assistant, or nurse practitioner, or licensed midwife caring for a pregnant student, certifies in writing that the student has a health condition which is a valid contraindication to receiving a specific vaccine, then a permanent or temporary (for resolvable conditions such as pregnancy) exemption may be granted. This statement must specify those immunizations which may be detrimental and the length of time they may be detrimental. Provisions need to be made to review records of temporarily exempted persons periodically to see if contraindications still exist. In the event of an outbreak, medically exempt individuals should be protected from exposure. This may include exclusion from classes or campus.

RELIGIOUS EXEMPTION

A student may be exempt from vaccination if, in the opinion of the institution, that student or student’s parent(s) or guardian of those less than 18 years old holds genuine and sincere religious beliefs which are contrary to the practice of immunization. The student requesting exemption may or may not be a member of an established religious organization. Requests for exemptions must be written and signed by the student if 18 years of age or older, or parent(s), or guardian if under the age of 18. The institution may require supporting documents. It is not required that a religious exemption statement be notarized. In the event of an outbreak, religious exempt individuals should be protected from exposure. This may include exclusion from classes or campus. In accordance with PHL Section 2167 and Title 10 New York Codes, Rules and Regulations Subpart 66-2 (10 NYCRR Subpart 66-2), the Academy distributes information about meningococcal disease and vaccination to students.

FAQ

If the student cannot provide acceptable proof of compliance, must the institution require re-immunization of the student?

If the student cannot provide acceptable proof of immunity or religious or medical exemption, then re-immunization is required. However, if the student cannot access his/her immunization record and the student can prove that he/she attended school in the United States after 12/31/80, one dose of measles vaccine can be assumed. If this option is used, the second dose must be administered no more than one year prior to attendance at an institution.

You can get low- or no-cost immunizations at the Health Department’s Fort Greene Health Center immunization clinic.

In order to comply with Medicaid rules, the Health Department’s immunization clinic must request insurance or payment for services. If you have health insurance, the Health Department will bill your insurance. You will not be charged a copay. If you are over 19 years of age and you do not have health insurance or do not want your insurance billed, the Health Department will ask for a sliding scale fee based on family size and annual income. Clinic staff will not ask for proof of family size and income, and services will be provided regardless of ability to pay.

The health centers charge a sliding scale fee payment based on family size and annual income. Clinic staff will not ask for proof of family size or income. Services will be provided regardless of ability to pay. You do not need to make an appointment. You will be seen on a first-come, first-served basis. Be sure to bring your vaccination record, if available.
