Academy identification cards (ID cards) are issued to all Faculty, staff and students. Faculty, staff and students are required to carry their ID cards at all times while on campus. Cards must be presented at the lobby security desk upon entry and to Academy staff and officials whose assigned responsibilities authorize them to seek proper identification upon request. Access to all areas of the building beyond the lobby is by keycard only. Academy ID cards function as keycards. This security entry system is part of the emergency notification program in the event of any emergency to ensure that all persons are accounted for. Students who have misplaced their ID cards must go to the front security desk, sign in, and leave a photo ID (such as a driver’s license) in exchange for a temporary card.
Please treat your ID card with care. Holes, creases, and other damage will make the card inoperable. Do not bend the card, leave in direct sunlight, near another heat source. Contact with magnetic fields will deactivate cards.
New ID Cards
ID Cards will be prepared prior to arrival on campus and distributed by the Director of Student Services during orientation. If you are a new student and did not submit a photo prior to your arrival on campus, please complete the form above.
The Academy requires one of these forms of identification when obtaining an ID card.
NOTE: Name on government authorized ID cards must match the name in the Academy’s database (no nicknames).

Replacement ID Cards
Lost, stolen or damaged ID cards should be reported immediately to the Registrar registrar@nyaa.edu. ID Cards will be replaced for a $10 fee. To obtain a replacement card, please complete the form above and pay replacement fee at the Office of Accounting.
Photo Specifications
You must provide one (1) photo that meet the following requirement:

- Dimensions – Image dimensions must be in a square aspect ratio (height must be equal to the width) – Minimum dimensions are 600 x 600 pixels; Maximum dimensions are 1200 x 1200 pixels
- Color – The image must be in color (24 bits per pixel) in sRGB color space which is the common output for most digital cameras
- File Format – The image must be in JPEG file format
- File Size – The image must be less than or equal to 1 Mb (megabyte) – If scanned, at a resolution of 300 pixels per inch
In addition to the digital image requirements, your photo must be:

- In color.
- 2 x 2 inches in size if printed.
- Sized such that the head is between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head.
- Taken within the last 6 months to reflect your current appearance.
- Taken in front of a plain white or off-white background.
- Taken in full-face view directly facing the camera.
- With a neutral facial expression and both eyes open.
- Taken in clothing that you normally wear on a daily basis: Uniforms should not be worn in your photo, except religious clothing that is worn daily.
- Do not wear a hat or head covering that obscures the hair or hairline, unless worn daily for a religious purpose. Your full face must be visible, and the head covering must not cast any shadows on your face.
- Headphones, wireless hands-free devices or similar items are not acceptable in your photo.
- If you normally wear prescription glasses, a hearing device or similar articles, they may be worn for your photo.
- Dark glasses or non-prescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required.)
- Glare on glasses is not acceptable in your photo. Glare can be avoided with a slight downward tilt of the glasses or by removing the glasses or by turning off the camera flash.
